The broader Keys stretch across Monroe County from Key Largo in the north through Islamorada and Marathon in the Middle Keys down to Key West itself, a single two-lane highway connecting island communities that can be an hour or more apart. That geography means dispensary storefronts are sparse and delivery times run longer than almost anywhere else in the state, with orders routinely scheduled rather than same-day the further up the chain a patient lives.

A limited number of MMTC storefronts serve the island chain directly, including a location in Key West itself, while patients in the Upper and Middle Keys often depend on delivery routed from either Key West or the Miami-Dade mainland depending on which is closer. No dispensary operates on Duval Street or directly in Old Town's historic core.

Florida legalized medical marijuana by constitutional amendment in 2016 and has never taken the recreational step since.
