New Orleans sits below the level of the Mississippi River behind an engineered levee system, and that geography shapes more than the city's drainage - tourism built on the French Quarter, the Port of New Orleans, and a healthcare and hospitality workforce that runs around the clock all feed a patient base with irregular schedules and a lot of visiting family. The metro carries two of Louisiana's ten base pharmacy permits, more than any other region in the state, plus the satellite network that comes with them.

H&W Acquisition Company LLC operates the original New Orleans permit at 1667 Tchoupitoulas Blvd, Suite B, near the Warehouse District, with satellite locations in Metairie (5055 Veterans Memorial Blvd) and across the river in Harvey (1544 Manhattan Blvd) on the West Bank. Crescent City Therapeutics LLC holds the metro's second base permit in neighboring Kenner and runs a satellite inside New Orleans proper at 1407 S. Carrollton Avenue, putting a second pharmacy option within the city limits itself. Out-of-state visitors with a valid medical card and qualifying condition from home can legally buy here too, under the 2024 reciprocity law, which matters in a city this dependent on tourism.

Between Tchoupitoulas Street near the Warehouse District and the Carrollton Avenue corridor uptown, these two retail clusters cover most of Orleans Parish without requiring a Causeway or river-bridge crossing for the majority of residents. Add the Metairie and Harvey satellites and greater New Orleans has more standing pharmacy access than anywhere else in the state, though a below-sea-level flood event can still knock a location offline faster than in higher-ground cities.

Louisiana calls its medical cannabis outlets "therapeutic marijuana pharmacies," and that is not a marketing label - it is the actual legal structure. Licensed pharmacists, not budtenders, staff the counter, and the whole program traces back to Act 261 of 2015. Recreational cannabis has no legal path in the state: a 2025 bill that would have created a temporary adult-use pilot program running from January 2026 through mid-2029 was referred to the House Administration of Criminal Justice Committee in April and never received a vote there, dying when the session ended that June.

Smokable flower has been legal since January 2022; before that, patients were limited to tinctures, metered-dose inhalers, and other non-flower forms.

2025 brought the biggest structural shift in years: Act 693 of 2024 moved retailer oversight from the Louisiana Board of Pharmacy to LDH's own Cannabis Program effective January 1, 2025, and an emergency rule that March formally renamed the license type from "dispensary" to "retailer" and locked in the 30-site cap. None of this touches the possession limit, which remains 71 grams of flower per patient per 14-day window, or the requirement that every purchase still starts with a physician's recommendation.

Running alongside the medical program is a separate, tighter hemp-derived THC track. Act 752 of 2024 caps hemp edibles at 5 mg of THC per serving, bans every inhalable hemp product outright, and restricts sales to buyers 21 and older at permitted retailers - never gas stations. A medical card is still the only legal way to buy real-potency flower in Louisiana.

Every product sold at a Louisiana pharmacy traces back to one of exactly two licensed manufacturers: Good Day Farm Louisiana LLC and Advanced Biomedics LLC, doing business as Ilera Holistic Healthcare. There is no third supplier and no independent craft-grow market - what varies pharmacy to pharmacy is simply which manufacturer's current harvest is on the shelf and in what form. The ten regional retailers carrying that supply include H&W (New Orleans, with satellites in Metairie and Harvey), Crescent City Therapeutics (Kenner), Capitol Wellness Solutions (Baton Rouge), Hope Pharmacy (Bossier City/Shreveport), The Apothecary Shoppe (Lafayette), Medicis (Lake Charles), Green Leaf Dispensary (Morgan City/Houma), Delta Medmar (Monroe), The Medicine Cabinet Pharmacy (Pineville, across the river from Alexandria), and Willow Pharmacy (Covington/Slidell). Ask any of them which manufacturer supplied a given batch and for that batch's lab results before buying.

The headline 2025-2026 story is regulatory, not retail: Act 693 finished moving retailer oversight from the Board of Pharmacy to LDH on January 1, 2025, the March 2025 emergency rule renamed dispensaries to retailers, and a January 2026 final rule locked the framework in. Meanwhile the adult-use pilot bill (HB 627) died without a committee vote when the session closed in June 2025, so the medical-only structure holds through the rest of 2026. On the hemp side, Act 752's inhalable ban and 5 mg serving caps have visibly thinned gas-station and convenience-store shelves since ATC enforcement began in January 2025. For any retailer, being the pharmacy patients can actually find - by name, address, and current hours - matters more in a ten-permit market than in a state with hundreds of storefronts competing for the same search.
