Mandan grew as a railroad town on the west bank of the Missouri, and it still runs on that industrial base: the BNSF yard, the refinery on the northeast edge, agriculture out of Morton County and the commuter flow into Bismarck. Roughly twenty-four thousand people live here, enough to make it the fifth-largest city in North Dakota, yet the cultural distance from the capital across the water is a point of local pride, reinforced every Fourth of July when the rodeo takes over town.

For cannabis, the river is a convenience rather than a barrier. Morton County holds none of the eight dispensary licenses the state allows, so patients registered with the Division of Medical Marijuana drive east across the Memorial Bridge to the Bismarck store. That trip is short enough that many Mandan residents treat the capital counter as their own. Everything else follows state rules: card holders only, no adult-use sales, no home cultivation and no delivery of any kind.

What Mandan has instead of a dispensary is the corridor leading to one. Memorial Highway runs east out of town toward the bridge, lined with dealerships, farm equipment lots and service businesses, and it delivers drivers directly to the licensed store on the far bank. North Dakota Statewide Context

North Dakota has been a medical-only state since voters approved the Compassionate Care Act as Initiated Measure 5 in 2016. The first dispensary sales did not happen until March 2019, and the program is run by the Division of Medical Marijuana inside the Department of Health and Human Services, based at 600 East Boulevard Avenue in Bismarck. The state counted 10,317 cards issued as of August 6, 2026.

August 1, 2025 brought the biggest expansion the program has had. House Bill 1203 and Senate Bill 2294 added cannabinoid edibles in the form of square hard or soft lozenges capped at 5 mg of THC per serving and 50 mg per package, dropped the in-person requirement for a first certification visit so telehealth now counts, stretched patient cards from one year to two, and raised the application fee to $40. Senate Bill 2293 capped concentrate containers at one gram. Nonresidents can now get a 60-day North Dakota card.

Politically the state has narrowed but not moved. Measure 5 lost 52.55 to 47.45 percent in November 2024, the third defeat in six years. Meanwhile Minnesota opened licensed adult-use retail in September 2025 and Montana has sold adult-use since 2022, which leaves North Dakota ringed by legal stores it gives its own residents no legal way to use.
