On November 12, 2021, more than four years after the Medical Cannabis Act was signed, Trulieve opened at 1397 Earl Core Road and roughly two hundred people queued up outside before the doors moved. Morgantown got the first permit largely because Monongalia County had the patient base to justify it, a function of WVU Medicine, Ruby Memorial Hospital and a university town population that skews younger and more mobile than the rest of the state.

That head start compounded. Monongalia County now supports nine permitted dispensaries and four separate national operators, which is unmatched anywhere in West Virginia. WVU itself is not part of that story. University policy prohibits cannabis on campus and in university housing regardless of a state card, and federal funding rules make that unlikely to change no matter what the legislature does.

The stores fall into four pockets. Don Knotts Boulevard, the riverfront stretch of U.S. 119 below downtown, holds Cannabist at 225 and The Landing at 63 within sight of each other. Sabraton on the east side carries the original Trulieve at 1397 Earl Core Road plus NewLeaf at 3227, with Zen Leaf on Venture Drive off Greenbag Road nearby. West Virginia Statewide Context

West Virginia sells cannabis to registered patients and nobody else. The Medical Cannabis Act became law on April 19, 2017, and the first legal sale did not happen until November 12, 2021, when Trulieve opened its doors on Earl Core Road in Morgantown. Pricing is unusually clean. The 10 percent tax lands on the dispensary rather than on you, and the statute forbids printing it as a separate line on a receipt, so the shelf price is the final price. The 2026 session was busy and unproductive: an edibles bill cleared the House and died in Senate committee, the home-grow and smoking bills never left their first committee, and Governor Patrick Morrisey vetoed the bill that would have redirected program fund money.
