How to Get Your Iowa Medical Cannabidiol Card

Iowa caps you at 4.5 grams of THC every 90 days. Everything else about the program follows from that number.

Iowa calls its program medical cannabidiol, and the name is a fair warning. This is not a dispensary state in the way Illinois or Missouri is. Registered patients buy from five licensed dispensaries, cannot have anything delivered, and work against a rolling 90-day THC ceiling that most patients in other states would use in a fortnight. It is still a real program with real products, and for the right conditions it works.

Qualifying Conditions

Iowa uses a defined list, and several entries require that the underlying condition or its treatment produces a specific symptom.

  • Cancer, where the condition or treatment produces severe pain, nausea, severe vomiting or wasting
  • Multiple sclerosis with severe and persistent muscle spasms
  • Seizures, including those caused by epilepsy
  • AIDS or HIV
  • Crohn disease
  • ALS
  • Any terminal illness with a life expectancy under one year, where it produces severe or chronic pain
  • Parkinson disease
  • Chronic pain
  • Severe intractable autism with self-injurious or aggressive behaviors
  • Ulcerative colitis
  • Corticobasal degeneration
  • Severe nausea or vomiting
  • Cachexia or severe wasting syndrome
  • Post-traumatic stress disorder

Chronic pain was added later than the original list and is now the most commonly used pathway. Because the 90-day THC ceiling is so low, discuss product form with your practitioner. A patient using tinctures at low doses gets far more out of 4.5 grams than one buying high-THC vape cartridges.

Finding a Certifying Practitioner

Iowa allows a wide set of health care practitioners to certify, and permits telemedicine, which matters in a state with five dispensaries and a lot of rural distance.

Your own practitioner

Standard office visit rate

Physicians, physician assistants, advanced registered nurse practitioners and podiatrists can certify within their scope. Your existing provider already documents the condition, which is exactly what the certification requires.

Certification clinics

Roughly 100 to 200 dollars

Practices around Des Moines, Cedar Rapids and Iowa City that handle certifications regularly. They know the portal and can advise on the waiver route if your condition needs more than the standard limit.

Telemedicine

Varies by service

Iowa permits certification by telemedicine with an approved practitioner, which removes a significant travel burden for rural patients. Confirm the practitioner is Iowa-approved before paying.

Tip: Ask about the waiver at the certification appointment, not afterwards. If your condition justifies more than 4.5 grams of THC per 90 days, the practitioner is the one who documents it, and doing it later means another visit.

The Application Process

Iowa runs an online registration through the Department of Health and Human Services, following a practitioner certification.

  1. Confirm Iowa residency and a listed condition

You need an Iowa driver license or state ID and a condition on the program list. Iowa does not register out-of-state patients.

  2. Get certified

See an approved Iowa health care practitioner in person or by telemedicine. They complete the certification form and, where appropriate, document a waiver for a higher THC limit.

  3. Register online

Complete the patient registration through the state portal, uploading your certification, ID and photo. Caregivers register separately and undergo a background check.

  4. Pay the fee

The registration card fee is 100 dollars, reduced for applicants who qualify on the basis of certain state assistance programs or a Social Security disability determination.

  5. Collect at a dispensary

Once your card is issued you can buy at any of the five licensed dispensaries. Bring your card and photo ID. Every purchase is drawn against your 90-day THC total.

Costs and Fees

Iowa charges more than most limited programs for the card itself, and the reduced rate is worth asking about.

  • Registration card 100 dollars
  • Reduced rate Available for qualifying applicants, including some on state assistance
  • Practitioner certification Office visit rate, or 100 to 200 dollars at a clinic
  • Caregiver registration Separate fee plus a background check
  • Annual renewal Same card fee, plus a fresh certification

Total Estimate: 100 to 300 dollars for the first year

After You Are Approved

The 90-day THC ceiling is the defining feature. Plan your purchases around it rather than around price.

  • Buy at any of the five licensed Iowa dispensaries with your card and photo ID
  • A rolling limit of 4.5 grams of total THC per 90-day period, tracked at the point of sale
  • A waiver route for patients whose practitioner documents a need for more
  • Products include capsules, tinctures, topicals, vaporizable forms and low-dose edibles
  • No smokable flower under the program
  • No delivery. Iowa Admin. Code 641-154.41(2) bars dispensaries from delivering
  • No lawful public consumption, and no protection from a workplace drug policy

Renewal

Iowa cards run one year. Renewal requires a fresh practitioner certification and another registration fee. Your 90-day THC allowance keeps rolling independently of the card date, so a lapse in registration does not reset it, it simply stops you buying until the new card issues.

Timeline: Start 45 days out. If you are using the waiver route, allow longer, because the practitioner has to document the justification again with the renewal certification.

Frequently Asked Questions

What is the 4.5 gram limit?

Iowa caps registered patients at 4.5 grams of total THC purchased in any rolling 90-day period. It is tracked at the register across all dispensaries, and it is the tightest limit of any active state program.

Can I get more than the limit?

Yes, through a waiver. Your certifying practitioner documents why your condition requires a higher amount. Raise it at the certification appointment rather than after you run out.

How much does the card cost?

One hundred dollars for a one-year registration, with a reduced rate available for qualifying applicants. The practitioner certification is separate.

Can I buy flower?

Iowa does not sell smokable flower under the program. Available forms include capsules, tinctures, topicals, vaporizable products and low-dose edibles.

Is delivery available?

No. Iowa Admin. Code 641-154.41(2) bars dispensary delivery to any location. Every purchase is collected in person at one of the five dispensaries.

Can I use telemedicine to get certified?

Yes. Iowa permits certification by telemedicine with an approved practitioner, which is a meaningful help given how few dispensaries there are and how far apart they sit.

Does Iowa accept out-of-state cards?

No. The registry is for Iowa residents, and out-of-state patients cannot buy from an Iowa dispensary.

Will a card protect my job?

No. Iowa employers can maintain drug-free workplace policies and act on a positive test regardless of your registration.
