The island's economy runs almost entirely on tourism — cruise ships docking downtown, Duval Street's bar crawl, and the sunset celebration at Mallory Square — and that visitor population overwhelmingly cannot buy under Florida's medical-only law regardless of how long they've been coming.

The broader Keys stretch across Monroe County from Key Largo in the north through Islamorada and Marathon in the Middle Keys down to Key West itself, a single two-lane highway connecting island communities that can be an hour or more apart. That geography means dispensary storefronts are sparse and delivery times run longer than almost anywhere else in the state, with orders routinely scheduled rather than same-day the further up the chain a patient lives.

Three MMTC storefronts serve Key West directly — two on Duval Street in Old Town and one on Flagler Avenue in New Town — and a fourth sits up the highway in Tavernier. Patients in the Middle Keys still depend on delivery routed from either Key West or the Miami-Dade mainland, depending on which is closer.
