When Farrington Highway closes for an accident or a rockfall, the coast is cut off, and that fragility shapes how households here plan everything from groceries to medical appointments.

Cannabis is no exception. There is no licensed counter anywhere on this coast, and none is coming, since the state has not issued new dispensary licenses since the original round. Patients drive to Kapolei, where two of the three Oahu licensees have stores, or further into town if they are already headed that way. From Makaha that is a real commitment of time, not a quick errand.

The community here has the highest concentration of Native Hawaiian residents on Oahu, along with real economic pressure, high housing costs relative to income, and a well-documented shortage of local medical services. The Waianae Coast Comprehensive Health Center is the anchor for care. Patients registering for a 329 card need a certifying physician or advanced practice registered nurse, and since 2025 that relationship may be established over telehealth, which removed a genuine barrier for this coast.

Commercially the coast is one long strip: Farrington Highway through Nanakuli, Maili, Waianae town and Makaha, with the Waianae Mall and the boat harbor as the main anchors. Any business serving this coast needs to be findable from that highway.

Hawaii Statewide Context

Hawaii passed the first state medical cannabis law written by a legislature rather than a ballot measure in 2000, then waited seventeen years for a store to open. Act 241 built the dispensary system in 2015, and the first legal sale happened on Maui in August 2017. Everything you can buy today flows through that system: eight licensed companies, each growing and manufacturing on the island where it sells, with the Department of Health Office of Medical Cannabis Control and Regulation running licensing, testing oversight and inspections.

Access starts with a 329 card. A physician or advanced practice registered nurse certifies a qualifying condition, you apply through the state portal, and registration costs $38.50 for one year or $77.00 for two. Act 241 of 2025 capped what a certifying provider may charge at $105 for a one-year certification and $210 for a two-year one, and it let providers establish the patient relationship over telehealth. Roughly 29,200 in-state patients held valid cards as of July 2026.

Cardholders may possess four ounces of usable cannabis and grow up to ten tagged plants at one registered site. Dispensaries cannot sell more than four ounces to any patient in fifteen consecutive days or eight ounces in thirty. There is no separate cannabis excise tax, so your receipt reflects the general excise tax, passed on at up to 4.712 percent in every county.

Two things trip people up. Cannabis cannot be carried between islands, not even by a patient with a valid card, because federal airspace and waters sit in between. And hemp is regulated hard: interim rules effective January 28, 2026 cap gummies at one milligram of THC per piece, ban hemp flower and hemp vapes outright, and require every retailer selling hemp products to register with the state.
