Hawaii sells cannabis to registered patients only. Eight licensed companies run stores on four islands, visitors with a home-state card can register in advance, and nothing may be carried between islands. Here is how buying actually works in 2026, island by island and town by town.

Overview
Hawaii passed the first state medical cannabis law written by a legislature rather than a ballot measure in 2000, then waited seventeen years for a store to open. Act 241 built the dispensary system in 2015, and the first legal sale happened on Maui in August 2017. Everything you can buy today flows through that system: eight licensed companies, each growing and manufacturing on the island where it sells, with the Department of Health Office of Medical Cannabis Control and Regulation running licensing, testing oversight and inspections.
Access starts with a 329 card. A physician or advanced practice registered nurse certifies a qualifying condition, you apply through the state portal, and registration costs $38.50 for one year or $77.00 for two. Act 241 of 2025 capped what a certifying provider may charge at $105 for a one-year certification and $210 for a two-year one, and it let providers establish the patient relationship over telehealth. Roughly 29,200 in-state patients held valid cards as of July 2026.
Cardholders may possess four ounces of usable cannabis and grow up to ten tagged plants at one registered site. Dispensaries cannot sell more than four ounces to any patient in fifteen consecutive days or eight ounces in thirty. There is no separate cannabis excise tax, so your receipt reflects the general excise tax, passed on at up to 4.712 percent in every county.
Two things trip people up. Cannabis cannot be carried between islands, not even by a patient with a valid card, because federal airspace and waters sit in between. And hemp is regulated hard: interim rules effective January 28, 2026 cap gummies at one milligram of THC per piece, ban hemp flower and hemp vapes outright, and require every retailer selling hemp products to register with the state.

What People Actually Pay
Hawaii is the most expensive legal cannabis market in the country, and geography explains most of it. Eighths commonly land between $40 and $65, grams between $12 and $20, and cartridges between $40 and $70, with the strongest discounting coming from weekday specials and loyalty points rather than price wars. Nothing gets trucked in from a cheaper state, electricity costs are the highest in the nation, and each island supports only two or three retailers. Budget the general excise tax on top of the shelf price.
How Access & Delivery Work
Hawaii never authorized cannabis delivery, and none of the eight licensees can legally hand product to a courier. Every transaction ends with a cardholder standing at a counter with a 329 card and a matching photo ID. What dispensaries do offer is online pre-ordering: you build a cart from the store menu, get a text when it is packed, and collect it during business hours. Most stores take cash and debit only, because card networks still will not process cannabis purchases. Patients on islands without a store on their side of the island simply drive; nothing may be shipped or flown, and mailing cannabis is a federal offense.

The November 2025 interim rules explicitly allowed inhalable oil extracts and concentrates that contain no nicotine, tobacco or hemp-derived cannabinoids, which widened the dab and cartridge shelf.
- Tinctures
Sublingual tinctures and drink boosters are a staple for older patients and anyone who cannot smoke; dosing is measured per milliliter on the label.
- Topicals
Balms, salves and roll-ons are stocked for joint and muscle complaints and do not count against your inhaled routine, though they still come out of your four-ounce allowance.

Only eight companies hold Hawaii dispensary licenses, and each sells its own cultivation under its own name. On Oahu you have Aloha Green Apothecary, Cure Oahu and Noa Botanicals. Maui County has Maui Grown Therapies and Pono Life Maui. Hawaii Island has Big Island Grown and Hawaiian Ethos. Kauai has Green Aloha. There are no third-party brands imported from the mainland, because nothing may cross state or county lines; what you see on a shelf was grown and packaged by that licensee or bought from another Hawaii licensee through an approved dispensary-to-dispensary transfer.
Current Trends (2026)
The 2026 legislative session ended the way the previous several did: the Senate moved a narrow low-dose adult-use measure, SB 3275, and it died at the March 12 crossover deadline after House leadership said the votes were not there. Regulators, meanwhile, were busy. The November 2025 dispensary rules loosened what stores may sell and let a caregiver sit in the waiting room, while Act 269 of 2025 forced every hemp retailer in the state to register, with enforcement starting July 1, 2026. Patient counts are drifting up slowly, not spiking.

Can tourists buy cannabis in Hawaii?
Only with paperwork. If you hold a valid medical card from another US state or territory, you can apply online for a temporary Hawaii registration up to sixty days before your trip. The fee is $49.50 and the card lasts a maximum of sixty days, renewable once per calendar year.
Does Hawaii have cannabis delivery?
No. State law never created a delivery license, so every purchase happens face to face at a licensed retail location. Dispensaries do let patients order online in advance and pick the order up, which is the closest thing available.
Can I fly with cannabis between the islands?
No. Hawaii law specifically excludes inter-island transport from the definition of medical use, so even a valid 329 card does not protect you at an airport screening checkpoint. Buy on the island where you will consume.
