Bismarck is the only city in North Dakota where the cannabis regulator and a licensed dispensary sit in the same zip code footprint. The Division of Medical Marijuana runs the entire eight-store program from an office on the Capitol grounds at 600 East Boulevard Avenue, under the tower that has defined the skyline since 1934. That means the paperwork side of being a patient is unusually visible here, and legislators who vote on the program drive past the retail reality every session.

The buying side is far more ordinary. Pure Dakota Health holds the Burleigh County license and sells to card holders who present the state ID along with a government photo ID. There is no home cultivation allowed anywhere in the state, no adult-use option, and no lawful way to have product brought to a house or apartment. Designated caregivers registered with the state remain the only workaround for patients who cannot make the trip themselves.

Geography puts the store in a useful place. Memorial Highway runs southwest from downtown toward the Missouri River bridge, a corridor already thick with dealerships, service businesses and the traffic moving between Bismarck and Mandan, so the dispensary sits on a route tens of thousands of people use daily. CBD and wellness retail is scattered along State Street north of Divide Avenue and around the Kirkwood Mall area downtown, all of it held to the non-intoxicating list this state allows and nothing stronger. North Dakota Statewide Context

North Dakota has been a medical-only state since voters approved the Compassionate Care Act as Initiated Measure 5 in 2016. August 1, 2025 brought the biggest expansion the program has had. House Bill 1203 and Senate Bill 2294 added cannabinoid edibles in the form of square hard or soft lozenges capped at 5 mg of THC per serving and 50 mg per package, dropped the in-person requirement for a first certification visit so telehealth now counts, stretched patient cards from one year to two, and raised the application fee to $40. Senate Bill 2293 capped concentrate containers at one gram. Nonresidents can now get a 60-day North Dakota card.

Politically the state has narrowed but not moved. Measure 5 lost 52.55 to 47.45 percent in November 2024, the third defeat in six years. Meanwhile Minnesota opened licensed adult-use retail in September 2025 and Montana has sold adult-use since 2022, which leaves North Dakota ringed by legal stores it gives its own residents no legal way to use.
